Please keep in mind that team registration is taken on a first come first served basis and the number of teams in each division is limited.



Step 1: Fill Out the New Team Questionaire

OR download the PDF, fill it out and fax it or mail it to us.

Step 2: Read the Following Documents

Step 3: Complete these Forms
Team Registration Form [PDF]
Team Manager/Captain Code of Conduct Acceptance Form [PDF]

And mail them with your $120 check for the team registration fee to SUASL (at address above).

There is a $120 non-refundable team registration fee. A refund will apply only if your registration arrives under sold out conditions.

Step 4: Have your Players Fill Out these Forms
Player Registration Form [PDF]
Player Code of Conduct Acceptance Form [PDF]

And mail them with their $80 check for the player registration fee to SUASL (at address above).
Make sure all new players to the SUASL league send in a photocopy of their drivers license and their payment with their registration forms.
We also require a digital head-shot be e-mailed to admin@suasl.com to SUASL to go on your player card that will be issued to the team manager once the player registration is complete.
